Evidence of Insurability (EOI)

As you're enrolling in certain insurance coverages, you may be required to complete an Evidence of Insurability (EOI) application* to provide current or past health information in order to be considered for coverage. When EOI is required, the insurance company needs to approve it before coverage becomes effective.


*Coverages that may require EOI: Supplemental group term life insurance and disability Income Insurance.
